Fees for services rendered ( patient portion if insurance is to be filed), are due at the time time of service unless other arrangenments are made in advance. Please let us know how we can best help you in this matter.
To help you with your financial needs we offer the following options for payment:
We gladly accept all Major credit cards,cash, checks, debit (as credit only)
We will gladly allow 2 -3 payments for services over $600.00
No Interest payment plans offered thru Care Credit allow you to pay over time for up to 12 months for plans over $300.00 with no annual fee or prepayment penalty
Extended low monthly payments are also offered thru Care Credit for 24-60 months for plans over $1000.00 with a low interest rate of 13.9%

Insurance
Our office is committed to helping you maximize your insurance benefits. We accept many PPO Plans, including Aetna, Metlife, Humana, Delta Premier, United Concordia, Cigna, Tricare Active Duty and Retire plan, Guardian, and Principle. We Do NOT accept and HMO or DMO plans. As a service to our patients, we will bill insurance companies for services, accept assignment, and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full. If you have any questions, our courteous staff is always available to answer them
Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. After insurance has paid any amount insurance may not have paid will be billed to you directly.
