Lady smilingFees for services rendered (patient portion if insurance is to be filed) are due at the time of service unless other arrangements are made in advance. Please let us know how we can best help you in this matter.

To help you with your financial needs, we offer the following options for payment:

  • We gladly accept all major credit cards, cash, checks and debit cards (as credit cards).
  • We accept 2 payments for patient portion amounts over $400 and 3 payments for patient portion amounts over $700.
  • We offer no-interest payment plans through CareCredit that allow you to pay over time for up to 12 months for plans over $300 with no annual fee or prepayment penalty. Extended low monthly payments are also offered through CareCredit for 24-60 months for plans over $1,000 with a low interest rate of 13.9%. You can review and apply for CareCredit in the office or online at www.carecredit.com.

Insurance

Our office is committed to helping you maximize your insurance benefits. We accept many PPO plans, including Aetna, MetLife, Humana, Delta Premier, United Concordia, Cigna, Tricare Active Duty and Retired plans, Guardian, and Principle. We DO NOT accept any HMO or DMO plans. As a service to our patients, we will bill insurance companies for services, accept assignment and allow them 45 days to render payment. After 60 days, you are responsible for the entire balance, paid-in-full.

Because insurance policies vary, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. After insurance has paid, any amount insurance may not have paid will be billed to you directly.
